NOTE: This column is a living document through Tuesday morning. I'll be updating as needed after team press conferences, imaging reports, Monday Night Football injuries, etc. Major changes to the content will be noted in red.
Scroll to the bottom of this article for a quick look table of the week's most pressing injury situations and a review of players on injured reserve / PUP / NFI.
Footballguys will have close and detailed coverage of all injuries this season. Here's a look at my expected schedule. I will do my best to stick to this. Work and family responsibilities may sometimes cause brief delays.
Monday Injury Rounds: Posted Sunday night, updated through Tuesday AM. Detailed analysis of the week's injuries with table for quick review. Offensive injuries will be live Sunday night. Critical defensive and offensive line injuries will be added throughout the day on Monday. I'll be updating with news and analysis through Tuesday.
Wednesday and Thursday Injury Expectations: Posted late Wednesday and Thursday evening. Analysis of the practice participation report with a table including my projection of each injured player's likely game status. Will include discussion of any major breaking injury news.
Sunday Gameday Expectations: Posted by Saturday afternoon, updated through Sunday morning inactives. Will include individual player blurbs with detailed thoughts on the week's injury news and my speculation on player availability and limitations. Will include table highlighting each day's practice participation, official game status, and my availability expectation.
I'll also be contributing to our waiver wire podcast (Tuesday release), our live Sunday morning pregame show (10 ET Sunday on YouTube), and plan at least one weekly Periscope Q&A (shortly before Monday Night Football) with additional shows if/when possible. As always, I will be retweeting key pieces of news and my own reaction and analysis, so make sure you're following @JeneBramel and checking my timeline on Twitter throughout the week.
INJURY ROUNDS
Julio Jones | Right hip flexor strain | Week-to-Week
Jones reportedly wanted to return to the game but could not generate enough explosion from his usual stance and the team chose to remain cautious rather than risk a higher grade injury. Jones will have an MRI this week to grade his injury. With the Falcons on bye in Week 5, Jones will have the benefit of intensive rehab this week rather than concern over game preparation.
Hip injuries are difficult to overcome for wide receivers, particularly flexor strains. Full strength is needed for smooth explosion off the line of scrimmage to defeat press coverage and to sink the hips to get in and out of route breaks. Should Jones' MRI show a low-grade strain, he'll have a good chance to rehab and return in two weeks.
***Monday Update: Adam Schefter confirms Jones has a hip flexor strain. A source told Schefter Jones is hopeful to return after the bye week.
Odell Beckham | Finger dislocation / High ankle sprain aggravation | Day-to-Day
Beckham's finger dislocation will be painful and probably prevent successful one-handed grabs next week but shouldn't otherwise be limiting. He also aggravated his preseason high ankle sprain in the middle of the fourth quarter and needed help from trainers before leaving the field. Though he returned for a late fourth quarter two-point conversion attempt and looked to be moving well, tomorrow's evaluation will be important. Hopefully, there's no significant swelling and stiffness overnight and the aggravation proves to be a non-issue.
Dalvin Cook | Left ACL tear | Out for Season
The Vikings suspect an ACL injury for Cook. Physical examination is almost always accurate for ACL injuries and the injury mechanism itself was highly suggestive of an ACL tear. Cook will have an MRI on Monday but the imaging study is more to assess surrounding structures for associated injury confirming the already-suspected ACL injury.
***Monday Update: The Vikings confirmed Cook's ACL injury today and Mike Zimmer noted concern for both meniscus and cartilage damage. ACL rehab can be complicated by cartilage damage but it's always hard to know the extent and location of those injuries. Cook will have nine months to prepare for training camp. If his rehab goes smoothly, he should return healthy in 2018.
Derek Carr | Back injury | Week-to-Week
Carr took a knee to the back and did not return. Adam Schefter tweeted he was told by a source Carr was suffering from back spasms and was expected to be okay. Jack Del Rio said the same in his post-game press conference. Carr didn't sound as optimistic and hinted he'd not been told the extent of his injury when addressing the media at his locker after the game.
I'm not willing to trust the day-to-day timetable just yet. While x-rays were negative, the team is worried enough about Carr's back spasms to send their quarterback for an MRI to look for additional soft tissue injuries. Mike Silver tweeted he was told Carr briefly experienced numbness in his legs after the hit to the area around his kidneys.
There's a lot going on in Silver's report. Numbness or weakness in the legs suggests more than simple back spasms and adds a layer of concern for nerve injury or spinal cord contusion. Whether that's accurate is debatable -- the quick reports of muscle spasm and minimal concern for next week are notable.
Hopefully, the Raiders will report reassuring MRI results on Monday. Until then, it's best to consider Carr questionable for next week.
***Monday Update: The initial day-to-day timetable on Carr did indeed prove incorrect. Carr was diagnosed with a transverse process fracture today and given an updated timetable of 2-4 weeks. This is the same injury suffered by Tony Romo (direct blow to back in a similar mechanism to Carr) and Cam Newton (injured in car accident). It wasn't misleading for Del Rio to say Carr had back spasms. He very likely did. This is a painful injury to the skeleton of the spine, made worse by spasm of the muscles that attach to the transverse process itself. Romo missed just one game with his injury, Newton missed two. The Raiders are hoping for Carr to also meet the more optimistic end of the recovery timetable.
Chris Carson | Left leg fracture | Out for Season
Carson left the field on a cart with his leg immobilized after a lower leg fracture and possible ankle dislocation on a late fourth-quarter run. Unless Carson's injury is much less severe than anticipated, he'll have surgery and be out for the season.
***Monday Update: Teammates sounded hopeful that Carson's injury wasn't as bad as anticipated after the game. Video replay remains concerning, however, and even a high-grade high ankle sprain would be a prolonged absence and potentially end Carson's season.
***Monday Update: The Seahawks confirmed a high ankle sprain w/ fibula fracture later today. Carson will have surgery, be placed on injured reserve, and miss the rest of the season. A full recovery in time for 2018 is expected.
Ty Montgomery | Rib injury | Week-to-Week
Reports on Montgomery's injury were all over the place after the game. Initial reports suggested Montgomery suffered multiple rib fractures, an injury usually requiring multiple weeks of recovery. Shortly after the game, Mike McCarthy told reporters he "didn't have a clear answer" about Montgomery's status. And Adam Schefter would later report Montgomery's injury wasn't as bad as initially feared. McCarthy seemed to confirm the more optimistic impression in vague terms the day after the game.
It's a weird set of reports and difficult to divine what may be true. I'm not certain why a specific report about multiple rib injuries would be shared before the x-ray result was known. And it's odd to have that specific report be incorrect after the x-ray unless something was lost in translation. We'll know more on Montgomery by midweek but prepare for him to be limited or inactive this week. The Packers' medical staff leans conservative and any chest injury -- see Randall Cobb in Week 3 -- carries a risk of aggravation and consideration of rest. On the positive side, Montgomery will have ten days to recover from the Thursday night injury.
Jamaal Williams | Left knee strain | Week-to-Week
The mechanism of Williams' injury could have been consistent with an LCL sprain but wasn't definitive. His MRI showed only muscle injury, however, which likely means minor damage to a hamstring tendon on the outside of his knee. Williams is considered questionable for this week's game but the extra few days of recovery after the Thursday night game could work in his favor.
Davante Adams | Concussion | Week-to-Week
Adams had the scariest injury of the week. Knocked out after a hit that sent his mouthpiece flying, Adams was immobilized and taken to the hospital for testing. Imaging studies and further examination reassuringly ruled out neck injuries. Adams was diagnosed with a concussion. Many NFL teams have been reluctant to move their players through the return-to-play protocol in one week this season. While loss of consciousness does not routinely translate to a longer recovery time, it's reasonable to expect the Packers to be cautious with Williams this week.
Marcus Mariota | Hamstring strain | Week-to-Week
Mariota was unable to return after a late first-half hamstring injury. The Titans don't appear to be too concerned about Mariota's injury but will send him for an MRI to better define his recovery timetable. The Titans have been cautious with players after hamstring injuries this season and the team was down 37-14 by the time they started their first drive of the second half. Expect continued caution with Mariota. He's one of the more mobile quarterbacks in the league and further aggravation would not only limit his mobility but could affect his downfield accuracy.
Others of note: Ameer Abdullah suffered a low ankle sprain during the fourth quarter and was reportedly cleared to return but didn't. Jordan Matthews had a large wrap over his right thumb on the sideline and would miss multiple weeks if he has a fracture or ligament injury. Monday Update: Matthews will have surgery to stabilize his thumb injury and will miss at least a month. Willie Snead is still recovering from preseason hamstring soreness and was not active in his first week after serviing his suspension. C.J. Prosise was inactive with an ankle injury that could require multiple weeks of recovery. Paul Perkins left with a rib injury and was ruled out.
FOLLOWUP APPOINTMENTS
Melvin Gordon | Knee contusion | Day-to-Day
Gordon's practice participation suggested he was healthier heading into Week 4 than Week 3. His performance suggested otherwise. Gordon didn't see a major decline in his offensive market share, with 10 of the team's 13 rushes and three targets. Playing through a painful bone bruise, Gordon may continue to look limited. Practice participation will be informative this week.
Andrew Luck | Right shoulder labrum repair | Week-to-Week
The Colts were hopeful Luck would return to individual work in practice last week but didn't clear him. During the Sunday night football game, Michele Tafoya reported Luck told her he expects to return to practice this week. I continue to believe Luck will need a minimum of two weeks of practice before he prepares to return to game action. That puts his earliest return at Week 7.
